Implement Solutions

In this phase, the implementation of the solutions begins, and the team starts to evaluate the extent of it. The evaluation of implementation includes the achievement of plan milestones, the review of pilot or small-scale implementations, and observation of effects (as specified in the Pathways to Results Implementation Plan Completion Worksheet). Recommended activities are:

1. Begin implementation steps in accordance with the plan.
2. Begin implementation of the data collection in accordance with the plan.
3. Review the data and assess the extent to which the solutions address the problem.
4. Document the results using the Pathways to Results Implementation Plan Completion Worksheet.
5. Modify the plan as necessary. This may include action items such as extending due dates, adding or deleting steps, or revising the responsibility of team members.
